Androgenetic alopecia (male and female pattern balding) is by far the most common cause of hair loss amongst men and a serious problem for many women. There are three important components which are responsible for both female and male balding:

1. A genetic predisposition for balding to occur.

2. Excessive presence of male hormones.

3. Aging – enough time for the first two factors to occur.

Both men and women produce male hormones that have a useful role to play in both sexes; but the fact that androgens occur in much higher concentrations in men explains why male pattern baldness is more common than the female balding.

DHT the root cause of hair loss
It is metabolism of male hormones (androgen/testosterone) which is main cause of hair loss and male and female pattern balding both in men and women.

The metabolism of androgen involves an enzyme called 5 alpha reductase which combines with the hormone (testosterone) and converts it to DHT (Dihydro-testosterone). DHT is a natural metabolite of our body.

The cause of male and female pattern balding
Some individuals, both men and women, are genetically pre-disposed to produce more DHT than the normal individuals. It is this accumulation of DHT and its effect on the cells inside the hair follicle and root which is one of the primary causes of male and female pattern balding.

When DHT gets into the hair follicle and root, especially a region called the dermal papilla, it changes the cell’ activity and prevents necessary proteins, vitamins and minerals from providing nourishment needed to sustain life in the hairs of those follicles. Consequently, hair follicles are reproduced at a much slower rate. This shortens their growing stage (anagen phase) and or lengthens their resting stage (telogen phase) of the follicle. DHT also causes hair follicle to shrink and get progressively smaller and finer. This process is known as miniaturization and causes the hair to ultimately fall. DHT induced androgenetic aloepcia is responsible for 95% of all hair loss.

Blocking the synthesis of DHT at the molecular level forms the basis for the treatment of MPHL (male pattern hair loss) and FPHL (female pattern hair loss). There are many natural DHT blockers and a number of drugs which are used for medical hair restoration.

Over the ages, baldness has been considered a disease. Lately, it has also been followed as a fashion. However, greater numbers of people still feel that a head covered by hair is always more attractive than a bald one.

Every month, hair grows by about 1 inch. Normally, around 85% of the hair on your head is always in the growing phase at any time, and 15% is not. Five years is the maximum amount of time for which hair normally lives. Considering these statistics, it is difficult to understand the causes of hair loss, which might be more apparent in a few of us.

The follicle is a cavity in the skin where the hair sits. In cases of hair loss this follicle shrinks over time, thereby causing shorter and thinner hair.  The final result could be a very tiny follicle with no hair in it. Usually the hair grows back, but for people who are already balding, the follicle is unable to grow a new hair.

Peladic is a disease which leads to hair loss. Some people are repelled at the sight of a bald head, and fear going bald themselves. This fear is called peladophobia.

In the dating arena, some women feel very strongly against men with bald heads, and insist that dating a bald guy is almost next to impossible. However, many women do not mind dating a man with a purposely shaved head. It’s almost like dating on the basis of your capability to grow more hair on your head.

A standard hair transplantation procedure involves the removal of donor strip of hair from the back of the head from where the follicular unit grafts are dissected under a microscope. These grafts are then preserved in saline and are then transplanted on a bald patch on the crown area of the scalp.

Given the time consuming and tedious nature of this procedure, a hair transplant surgeon is often able to transplant only about 500 to 600 follicular unit grafts per day. However, thanks to the recent hair transplant advances, this technique is often replaced by the follicular unit extraction (FUE). The cost per graft of FUE is typically twice the cost of the standard follicular unit hair transplant procedure discussed above but it is comparatively faster and minimally invasive.

Some hair transplant advances

In a typical FUE procedure a small round punch is made in the donor area to directly extract 1, 2, 3 and 4 hair follicular unit grafts. The follicular units extracted using this process is typically referred to as “blunt dissection” where a punch is made to envelope the entire follicular unit separating it from the surrounding soft tissues.

Once the underlying follicular unit is separated from the surrounding tissues, it is easily extracted using a small forceps. The small holes left behind after the follicular unit extraction gradually heal over the next few days and are not detectable to the naked eye once the patient’s hair grows out. The healing time is much less than the donor strip extraction procedure.

While the FUE procedure has been adopted by most hair transplant clinics, the standard strip excision method is still the most popular hair transplant procedure because it is more economical than the FUE.

Moreover, due to some recent hair transplant advances such as the use of trichophytic closure technique, the linear donor scar created by the strip excision procedure is now often rendered almost undetectable to the naked eye. This advancement in the hair transplantation technique has made FUE procedure relatively less appealing.

Hair transplantation research

Besides the hair transplant advances in the transplantation procedures, a lot of research is also underway to clone the hair. If the hair transplantation research is successful it will be possible to make several copies of donor hair in the laboratory. The application of this technique would be used in the form of hair transplantation. In the traditional procedures, the biggest limitation is often the donor which is not able to meet required density. However, hair cloning promises to overcome this problem by having the amount of hair required to be grown in a laboratory from a single donor hair and then implanting it into the scalp.

Hair transplantation research for cloning is very difficult and there are many hurdles that have to be overcome regarding the safety and cosmetic characteristics of the cloned hair. Some hair transplantation research has also provided breakthrough for some hair loss medication like Dutasteride.

Out of the hundreds of procedures he performs each year, the last thing hair-transplant surgeon Dr. Robert Dorin expected to do was perform surgery on his father.

Robert M. Dorin Sr. had been losing his hair for many years, but few people knew how much it bothered him. Then, one day at a family function, the elder Dorin casually mentioned to his son that he was considering treatment.

“It surprised me because my father knew what type of work I do and he never said anything,” Dorin said.

Caught unawares, Dorin was nevertheless delighted that his father was taking matters into his own hands. “My father’s hair loss finally reached a point where most of the hair in the frontal area had fallen out,” he said.

So Dorin decided to come up with a surprise of his own.

“Given that it was a week before my dad’s birthday, I figured it was a great opportunity to give him a gift that he would enjoy for the rest of his life,” he said. “I scheduled a procedure for my father for the very next week.”

On the day of the surgery, the two discussed the details of the procedure and what to expect.

“I could feel and see my dad’s excitement grow as things took shape,” Dorin said. “He realized just how much hair he had lost over the years and how much I would be able to put back in just one procedure.”

The procedure required 1,571 hair-transplant grafts and took about 7 hours. Three weeks later, there are no telltale signs of the hair transplant. He should begin enjoying his new look in three to six months.

“Working on my dad was something extra special for me,” Dorin said. “It was an honor to be able to do something that would have a lasting, positive impact on his life. It is very similar to the way his actions and support have shaped my life.”

The decision of when to perform a laboratory evaluation or a hair analysis test in women experiencing hair loss should be made on an individual basis by the hair restoration specialist. In this article we briefly describe some of the situations where a hair restoration doctor may order lab tests. It is important to note that this is just an overview for your general information so you can understand the reason and the result if you are asked for hair analysis or laboratory tests.

When a hair loss specialist or physician orders some laboratory tests, these hair analysis tests are ordered only after specific clinical information or after observing and carefully evaluating your hair loss. Understanding the basis of these hair analysis tests helps you to understand the cause of your hair loss and also helps you to make an informed decision for a particular hair loss treatment.

While hair loss directly due to a deficiency in a nutrient is believed to be rare, a large industry has developed in recent years based on nutrient “hair analysis”. Several clinics and laboratories claim to be able to define a deficiency of a nutrient or vitamin through analyzing a sample of your hair. The Internet enables these clinics and laboratories to advertise worldwide and a quick search should provide you with numerous web sites offering hair analysis.

These investigations have a simple yes – no question, is the drug, toxin, or heavy metal there or not? There is also the advantage that most of the metals and toxins looked for have a fairly stable chemical nature. They are un-reactive compared to nutrients so they are less likely to change with exposure to the environment. With nutrient analysis the laboratory is attempting to define shades of gray – exactly how much of the nutrient is there? This has been shown to be virtually impossible to do. Even sending hair samples from the same donor to different analysis labs yields contrasting results.
Each vitamin and most minerals are present in blood in extremely small concentrations. Blood also contains a great number of chemicals and molecules and many of these tend to interfere with vitamin and mineral tests. For this reason, a procedure like HPLC (high pressure liquid chromatography) that separates the vitamin or mineral from contaminating substances is usually performed prior to conducting the actual test.

Following separation or purification by HPLC, the vitamin or mineral is detected by a color reaction or fluorescence reaction. In these reactions, the amount of color or fluorescence that is formed is proportional to the amount of vitamin or mineral in the sample, allowing the amount of nutrient present in the original sample to be calculated. In the case of some nutrients, the purified factor of interest is reacted with a special chemical (reagent) prior to detection.

Hair analysis helps the hair loss specialist to reach a diagnosis on hair loss. There can be several Causes of Hair Loss in Women and Hair Loss in Men and after a correct diagnosis is made, the correct women hair restoration treatment can be suggested.
This is not to say all hair analysis is invalid. Analysis of hair is a very useful method of defining exposure to heavy metals. A hair analysis can reveal whether a person has had a chronic exposure to chemical toxins in the environment. And of course hair analysis can be used to show that an individual has been using illegal drugs. The test is known as Hair Follicle Drug Testing. These forms of hair analysis are looking for the presence of a particular chemical that is not normally found in hair, or looking for expression over and above that which is known to be safe.

While Androgenetic Alopecia is the number one reason why individuals experience hair loss, it is not the only one. Medical conditions such as hypothyroidism, ringworm and fungal infections can cause hair loss. Certain medications such as blood thinners, gout medication, birth control pills and too much vitamin A can cause sudden or abnormal hair loss as can following a crash diet, sudden hormonal changes, chemotherapy and radiation. If you are taking prescription medications, talk to your doctor and find out if your medication is contributing to your hair loss.

At any given time 10% of our hair is in what is called a “resting phase” and after 2-3 months resting, hair falls out and new hair grows in its place. Some people, however, experience more hair loss than is normal.

Avoid mega-doses of vitamin A. Too much vitamin A can cause your hair to fall out.

Exercise, do yoga, meditate or find some other practice that will help to reduce your anxiety and stress levels. If you wear pigtails, cornrows, use a curling iron, hair dryer or hot rollers, try changing your hair style to one that puts less pressure and stress on your hair and scalp.

If hot oil treatments or chemicals such as those used in permanents are causing inflammation to the scalp, discontinue their use, or reduce the number of times you are using them.

Use gentle shampoos and conditioners to avoid any unnecessary damage to your hair.

In men, herbs such as saw palmetto and licorice root help block the formation of DHT. The same holds true for supplementation with zinc. As an added benefit, studies show that these supplements can also help prevent prostate enlargement.

Massage your scalp with rosemary oil in an olive oil base. Both rosemary oil and massaging the scalp can stimulate the circulation in the scalp and promote hair growth. Again, if you are experiencing hair loss, check with your doctor to ensure that a more serious disorder isn’t the cause.

Not surprisingly, hair loss myths are more well known than hair loss facts.  Since hair loss is such a common and unwelcome condition, the public has been flooded with hair loss myths to promote sales of hair loss prevention products.  Many companies have jumped on the hair loss bandwagon because of our own enormous desires to eliminate hair loss from our lives, not to mention the unlimited amount of money that we (the public) are willing to spend on this quest.  This makes the hair loss market ripe for perpetuating myths that will increase sales.  But, for the most part, they are just that, myths.

Some of the more common hair loss myths are as follows:  Male Pattern Baldness comes from the mother’s side of the family and skips a generation.  Not true.  There is no one single gene that causes baldness.  Most researches believe that MPB is a result of several different genes, inherited from both parents, interacting with each other to cause hair loss.

Hair loss myth #2:  Pattern baldness affects only men.  Just ask any women suffering from pattern baldness, it’s not true.  In fact, hair loss is just as common in women as it is in men, it just usually occurs in a less virulent form and is easier to hide.  Also, women’s hair tends to thin out over the entire scalp area, whereas men’s hair thins in patches and at the forehead.

Hair loss myth #3:  Poor blood flow to the scalp area causes hair loss.  This is a misconception that has been perpetuated by companies selling hair loss products for years.  Ask any dermatologist and they will tell you, bald scalps have just as much blood flow as scalps full of hair.  It is because of this blood flow to the bald scalp that hair transplants work so well.

Hair loss myth #4:  If you haven’t lost your hair by 40, you aren’t going to.  Again, not true.  Age has nothing to do with it.  If you are genetically predisposed to loose your hair, you are going to.  Just be grateful you made it to 40 with your hair still intact.

You might be surprised to learn that many people are unable to untangle fact from fiction when it comes to hair loss. Some of the blame lies with the many companies and individuals exploiting hair loss myths in order to sell bogus hair products. Other myths may linger because people with hair loss, particularly women, are reluctant to talk about an issue that remains sensitive and sometimes emotional. The timing and pattern of pattern baldness is also different in men and women.

While men tend to start losing hair in the 30s and 40s, hair loss begins in the 40s or 50s in women, though it can occur as early as the 20s. And while men first lose hair in the front and at the top of the head, women’s hair thins diffusely throughout the scalp.

Until recently, women have been reluctant to seek treatment, but hair restoration surgeons say that women make up more and more of their practices. In reality, hair loss is just as common in women as it is in men, though the degree of loss tends to vary by gender. ‘By the age of 50, over 50 percent of men have significant hair loss.’ Dr. McAndrews says.

‘For women, about 25 percent have significant hair loss by the age of 50, though it may be less apparent because women are more conscientious about hiding it than men are.’ For most people, hair loss is a result of a genetic predisposition. For some, however, hair loss may result from a condition called telogen effluvium. Telogen effluvium is a condition where a lot of hair may fall out in a relatively short period of time.

The most common causes are childbirth, high fever, severe illness and even high stress. In most cases this type of hair loss is temporary. Other common causes are like inadequate protein or iron in your diet, or poor nourishment caused by fad diets or an eating disorder. Chemicals used for dying, bleaching, straightening or perming hair as well as excessive hairstyling or hairstyles that pull your hair too tight can also cause gradual hair loss.
